First of all, if you wear glasses, get those screen protection lenses. If you don't, get non-powered screen protection glasses.
Take breaks from screen, every hour or 2 hours. Avoid screentime where it isn't necessary. Use warm colours for screen.

I have always emphasized this to my colleagues and friends to take care of their eyes. You can start with small things. Pick one eye drop (preferably Drishti, netra sudarshan) and use twice or once (at least) a day. Wash your eyes frequently, like once in the afternoon, once in the evening and before sleeping at night. Do eye exercises, try to look far , things like that. All these thy on a regular basis.

As someone who's wearing glasses for almost entirety of life, let me tell you that you can buy eye glasses with the best lenses in the world, hell you can even somehow use a diamond to make the glasses. But unless you give your eyes rest, nothing will work.
If you keep on walking and walking, at some point your legs will be tired and will tremble. So you will sit down or take some rest before you go forward.
Our eyes are same. The best way to protect them is to limit looking at anything which emits too much light like screens or phones or even a bulb. Stare at a wall, stare at your bathroom. Just give your eyes rest, close your eyes for 10 mins.
I personally get up and go on a walk twice between my work for 20 mins. Followed by 10 mins of shuteye. But that's just me managing my work along with the walk

I get a lot of hate to say this but the patanjali drishti eye drop helped me drastically. A lot of people say it has ginger garlic and what not, I'll go blind etc but I've seen people using it for more than 4-5 years going on perfectly. My eyes used to get all watery after every couple of hours of screen time, always itchy but all of it is gone and I can easily bear the 1 min discomfort you get while putting it in eye.

iPhone has this feature where it screams at you if your phone is too close to your eyes. It’s pretty good, the whole screen goes blank with the message and it won’t show you the content until your move to a safe distance.
